As a result of a printing error on the new Face-to-Face Life Insurance and Critical Illness Insurance Application Form, 126E (2016/10/01), there is a serious omission of “No” tick boxes for Questions 2b and 3 under Section 7 – Insurance History.

Replacement Forms Available January 10, 2017

Replacement forms 126E (2016/10/01) will be available for MGA order through BMO Insurance DATAOnline on January 10, 2017.  Upon receipt, please destroy your current inventory.

In the interim, the current form can be used but please ensure that questions 2b and 3 under Section 7 have been answered by ticking “Yes” or writing “No”.
